Abstract This paper presents the failure analysis conducted in collaboration with the maintenance department of an oil refinery for a period of three years. The item selected for the study represents one of the most important and diffuse components in the plant: the heat exchanger. During the work have been collected and analyzed data on all the failures happened in the selected equipment: types, quantities, locations with the aim of creating an objective classification for facilitating the emission of the work orders (WO). The results of the work have been the complete codification of all the failure effects, related to the selected equipments, and the determination of their performance index during the considered period of time.
